Your First Visits to Our Office

We are excited to meet you and have you become a member of our practice.

We are also excited to work with you to help you achieve your health objectives and create a better health future for you and your family.

The first thing we ask you to do is call the office at 760-758-HEAL (4325) and set up an appointment time that works for you. We have special times set aside for new practice members.

Don't forget to download and print out one of the following two forms (the Health Profile form relevant to your visit) to save time during your first visit here.

Please fill this form out completely and bring it with you to your first visit.

Please fill out this form for your child before bringing them in to be examined.

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office. You can print it out and bring it in to our office or Click Here to register online! The online newsletter sign-up is also on the right.
